I will create the scored form using one of our PDFs. Each statement needs to be provided to the survey respondent and then they must answer 1, 2, or 3 for each statement. After the survey is complete, a calculation needs to take place (see PDF for how that occurs) and the top three scores need to be shown to the respondent with the category of those top three scores. A legend of the scores (as included in the PDF) needs to be presented to the respondent as well.
